This applies to all drugs requiring PA, either because they are non-preferred drugs on the Preferred Drug List or because they are subject to clinical edits. A Formulary lists the brand or generic name of a given drug. If a medication does not appear on the Formulary, the medication is not covered under the pharmacy benefit. In some instances, a medication may require a prior authorization. The Formulary is searchable online.
