Blue Cross Medicare Advantage^SM Prior Authorization List Effective Jan. 1, 2016

The Blue Cross Medicare Advantage Prior Authorization List for 2016 was recently posted to the Medicare section of the Blue Cross and Blue Shield of Oklahoma (BCBSOK) provider website. This Prior Authorization List will be effective Jan. 1, 2016. The attending physician must obtain prior authorization for the services outlined in the Blue Cross Medicare Advantage Prior Authorization List, except in an urgent situation.
