Hypertension Resources

According to the Centers for Disease Control, about 1 of 3 US adults have high blood pressure.¹
Blue Cross and Blue Shield of New Mexico (BCBSNM) offers resources for providers who manage Blue Cross Community Centennial patients with hypertension and related illnesses:

  • Care Coordination and Disease Management Programs are available benefits for all members. You can refer your Blue Cross Community Centennial members for one-on-one hypertension education and support by having them call 1-877-232-5518, option 2.
  • Clinical Practice Guidelines (CPGs) distill national recommendations down to clinical guides and are updated at least every two years and reviewed by practicing New Mexico physicians. CPGs are meant to serve as general guidelines and are not intended to substitute for clinical judgment in individual cases.
  • The 2015-2016 Hypertension CPGs include recommendations to patients to achieve and maintain a goal blood pressure value.
  • The Quality Provider Toolkit contains a collection of concise resource tools for the busy provider. Topics include the controlling blood pressure quality measure and information about member incentives to promote healthy behaviors, such as a walking program.   • Notification letters to providers from Prime Therapeutics regarding the potential underutilization of medication, such as patients not refilling their anti-hypertensive medication routinely, may be received via fax throughout the year.
