Address Changes for Claims, Appeals and Overpayment Recovery Requests

Effective December 31, 2015, the address for claims and appeals submissions changed. It is critical that the following addresses are used for claims and appeals submissions:

Medicaid claims should be submitted to:

Medicaid appeals should be submitted to:

Beginning August 1, 2017, the U.S. Postal Service will no longer forward mail from the previously used P.O. Box 684787. All misdirected mail will be returned to sender, further delaying processing of any claims.

Overpayment Recovery Address
Effective June 1, 2017, the address below will begin populating on future recoupment/overpayment letter requests. Providers should use this address for any check payments that are sent to Blue Cross and Blue Shield of Texas.
