Fully Insured Mid-Market Group Business: No Longer Need Individual Health Questionnaires

Occasionally, Blue Cross and Blue Shield of Illinois (BCBSIL) group markets required Individual Health Questionnaires for risk evaluation in the fully insured mid-market segment. Mid-market is defined as employers who employed on average more than 50 “total” employees (full-time, part-time, seasonal) on business days during the preceding calendar year; and enroll less than 151 employees.

Moving forward, Individual Health Questionnaires are no longer needed to assist in the risk evaluation of fully insured mid-market groups. 

The Illinois Standard Health Employee Application, which includes the Individual Health Questionnaire, has been removed from Blue Access for ProducersSM. It has been replaced by the 2017 Group Enrollment Application and Policy Change form. The 2017 form can be used for all small group, mid-market and large group business. Membership enrollment using our census spreadsheet templates will continue to be accepted. 

Other documents and forms will be updated to reflect this change in guideline. The new versions will be posted to Blue Access for Producers as they become available. 

This update only applies to fully insured business.
