HMO Network Update: Two Medical Groups Leaving the Network Jan. 1, 2014

The following medical groups will be leaving the HMO networks. Members who have questions should call the number on the back of their membership ID card for more information. Please contact your Blue Cross and Blue Shield of Illinois representative if you have questions.

Northwest Suburban IPA Swedish Covenant #348
Northwest Suburban IPA Swedish Covenant Medical Group #348 will no longer participate in the HMO Illinois and BlueAdvantageSM provider networks. The termination will become effective Jan. 1, 2014, and we will begin notifying impacted members this week.

Members will be given the opportunity to select new medical groups. Our transition of care plan will also be put into place so we can ensure that members’ treatment will not be negatively impacted and that we have appropriate time to transfer membership to other groups.

It is important to note that this development affects HMO members only, and only services provided by Northwest Suburban IPA Swedish Covenant. This does not affect our PPO members or contracts.

Sherman Choice Medical Group #359
Sherman Choice Medical Group #359 will no longer participate in the HMO Illinois and BlueAdvantage provider networks. The termination will become effective Jan. 1, 2014, and we will begin notifying impacted members next week.

Members will be given the opportunity to select new medical groups. We are including a physician crossover list with the member letters. Most of the impacted physicians will still be available to HMO members through another medical group. Our transition of care plan will also be put into place so we can ensure that members’ treatment will not be negatively impacted and that we have appropriate time to transfer membership to other groups.

It is important to note that this development affects HMO members only, and only services provided by Sherman Choice. This does not affect our PPO members or contracts.
