Announcing Care onTargetSM – Self-Service Online Condition Management for Members

Blue Cross and Blue Shield of Illinois (BCBSIL) is excited to announce Care onTarget, a dynamic new condition management tool that your employees can use to learn about and manage their conditions.

Care onTarget complements the robust catalog of health and wellness tools already available to members of BCBSIL. Members have access to the following:

  • Condition Assessments (CA): Online CAs allow members to answer basic questions about their health with their answers stored in one secure place. They can take CAs for asthma, coronary artery disease, diabetes, depression and substance abuse.*
     
  • Online Health Tutorials: Based on answers to the CA, Care onTarget will suggest online tutorials to help members better understand their care needs and offer ways for them to take a more active role in their care. Members also have access to the full tutorial library which covers a broad range of health topics, many in Spanish.
     
  • Health Resources: This section allows members to obtain useful health information from well-known and recognized groups such as the National Institutes of Health (NIH) and Centers for Disease Control and Prevention (CDC).
     
  • Live Chat with a Clinician: Members can chat with a clinician Monday through Friday, 8:30 a.m. to 5:00 p.m. Central time, excluding holidays.

Members can log on to Care onTarget directly at careontarget.com or by choosing the link in the My Health tab in their Blue Access for MembersSM (BAM) account. New users can access BAM with the user name and password created during the Care onTarget registration process.

*Depression and substance abuse assessments are only available to members whose employers have purchased the Behavioral Health program through BCBSIL.
