Affordable Care Act – Summary of Benefits and Coverage

Requests Must Be Submitted to Change Summary of Benefits and Coverage
The Summary of Benefits and Coverage (SBC) provision of the Affordable Care Act requires Blue Cross and Blue Shield of Illinois (BCBSIL) to use a government-developed template and to follow specific rules when creating SBCs. Because BCBSIL may be responsible for the accuracy of the SBCs, we do not allow any unauthorized changes to any portion of the document or to any elements in the document, including logos or fonts. If a change is necessary, BCBSIL must make it.

Generally, the only information that BCBSIL will routinely customize are the “Coverage Period” (dates) and “Coverage For” (e.g., individual or family) sections at the top right on Page 1. Follow these steps to request a change to these sections:

  • Contact the Summary of Benefits and Coverage Hotline at 855-756-4448.
  • Provide the Corporate ID (for example, Blue Cross and Blue Shield of Illinois) and Plan ID for the requested SBC.
  • Communicate the item(s) that needs to be changed (“Coverage Period” and/or “Coverage For”) and the new information.
  • Provide an email address for delivery of the updated SBC.

Always retrieve SBCs from the SBC Tool (https://ha.pivot.rrd.com) to ensure that you have the most current and accurate SBC.
