Use the Current BCBSIL Medicare Supplement Form [Individual Markets]
Effective Jan. 1, 2010, old Medicare Supplement Form OB3972 Rev 1/06 will be discontinued.
To consolidate our sales channel marketing materials, last year Blue Cross and Blue Shield of Illinois (BCBSIL) introduced a new version of the Medicare Supplement Application [form number 31222]. While we continued to accept applications submitted using the old Medicare Supplement form [form number OB3972 Rev 1/06], beginning Jan. 1, 2010, only the current form will be accepted. Applications submitted using the old form will not be processed.
We strongly encourage you to use the current form to submit new Medicare Supplement business. This updated form is being distributed in print (bundled in the BCBSIL Medicare Supplement producer enrollment kits) and is also available online on Blue Access for Producers.
