The personalized Health Risk Assessment (HRA) tool helps you learn about your health and your health risks by completing a questionnaire that takes just 10-15 minutes to complete! The tool evaluates your health through a holistic approach that takes into consideration such factors as stress, sleep, fitness and nutrition. Completing the HRA gives you a report outlining your health risks and strengths, as well as suggestions for healthier living that you can discuss with your physician.

And after you complete the HRA, you will be entered into a second drawing to win an iPod Shuffle or iPod Nano*!

iPod Nano/iPod Shuffle/iTunes Gift Card Promotion Details

AT&T employees and spouses who register or log in to BAM between February 19, 2010 and March 26, 2010 are entered into a drawing to win a $25 iTunes Gift Card. AT&T employees and spouses who log in to BAM between February 19, 2010 and March 26, 2010 and complete an HRA are entered into a second drawing for an iPod Shuffle or iPod Nano.

*One iTunes Gift Card will be given away. One iPod Shuffle will be given away for the first 25 HRAs completed. One iPod Nano will be given away for an additional 75 HRAs completed. A second iPod Nano will be given away for an additional 100 HRAs completed (two maximum). Those employees and spouses who have completed the HRA since January 1, 2010 will also be eligible for the iPod incentive. Your odds of winning will depend on the total number of entries received. The prize value will not exceed $150. No purchase is necessary. A purchase will not improve winning. Shipping and handling will not be the responsibility of the winner.
